Program Overview
MOQC has partnered with one medical oncology site from each of our regions and one gynecologic oncology site to offer 50 patients and their caregivers up to 14 delivered meals/week for 4 weeks along with educational materials.
Meals are provided through a partnership with Mom’s Meals.
Program Eligibility
To be eligible to participate in the Comfort Cuisine program, patients must:

- Have a cancer diagnosis AND
- One of the following:
- Screen as food insecure (Using the Hunger Vital Signs 2-question screening tool)
- Enrolled in Medicaid
- Low-income (less than 150% of the Federal Poverty Level)
MOCQ will complete the screening with patients after they have been referred.

Program Enrollment
To enroll patients, you must be a provider at one of our participating practices on the map (See right).
Steps to Enroll Patient(s)
- Identify a patient that meets criteria.
- Provider and patient completes referral form. The following information is needed:
- Provider name, phone, email, and MOQC region
- Patient name, address, DOB, phone number, email address, preferred language, and gender
- Meal options: Meals only, oral nutrition supplements only, or combination
- Caregiver meals (optional)
- Desired menu type, allergens, and other meal preferences
- Patient completes a baseline questionnare.
- Provider submits the Comfort Cuisine referral form and baseline questionnaire to MOQC for review via secure email (moqc@moqc.org) or secure fax (734-998-1447).
- MOQC submits patient and provider information to Mom’s Meals.
- Patient is screened and then contacted by Mom’s Meals to select their preferred meals.
